What is CDPAP?
The Consumer Directed Personal Assistance Program (CDPAP) is a transformative Medicaid-funded initiative in New York State that allows individuals in need of home care services to take direct control of their care. Unlike traditional home care programs where the agency assigns a caregiver, CDPAP empowers the consumer to select, train, and manage their own caregiver—often a trusted family member, friend, or neighbor.
At With Grace We Care LLC, we are dedicated to guiding individuals and families through every step of the CDPAP journey. From initial eligibility screenings to enrollment and caregiver onboarding, our team ensures a smooth, compliant, and compassionate process. We advocate for your right to choose care on your own terms—and we do it with grace, every step of the way.
CDPAP Eligibility Requirements
To be eligible for the Consumer Directed Personal Assistance Program (CDPAP), individuals must meet certain criteria:
Enrollment in a Managed Care Plan (if applicable)
Most applicants will need to be enrolled in an MLTC or other Medicaid-managed care plan to receive CDPAP services. Enrollment is based on the type of Medicaid coverage the consumer has. If you or your loved one meets these requirements, you may be eligible to hire a family member or friend as your caregiver, paid by Medicaid under CDPAP.
Assessment by a Medicaid-Approved Provider
The consumer must undergo a nursing and social assessment to determine, which is usually done through a Managed Long Term Care (MLTC) plan or Mainstream Medicaid Managed Care plan:
- The level of care required
- Approval for CDPAP services
Self-Direction Capability
The consumer must be:
- Mentally and physically capable of making decisions about their care
- Have a designated representative (family member, legal guardian, etc.) who can act on their behalf to recruit, train, and manage the caregiver.
Medicaid Enrollment
The consumer (person receiving care) must be actively enrolled in New York State Medicaid.
Note: Medicare alone does not qualify. If the consumer is not yet on Medicaid, we can assist with the application process.
Need for Home Care Services
The consumer must require assistance with daily living activities or skilled nursing care. This includes help with tasks such as bathing, dressing, mobility, medication management, and more.
