CDPAP Eligibility Requirements
With Grace We Care can make eligibility simple to understand and easy to navigate, guiding you step by step with patience, compassion and care. To participate in the Consumer Directed Personal Assistance Program(CDPAP), you or your loved one must:
Be enrolled in a Medicaid Managed Care or Managed Long-Term Care(MLTC) plan
Need help with daily activities such as bathing, dressing, moving around, taking medication and eating
Complete a nursing and social assessment to confirm the level of care needed.
Be able to make decisions about your care - or have someone you trust (a family member, friens or guardian) who can make those decisions for you.
Have a representative who can recruit, train and oversee your chosen caregiver if you cannot do so yourself.
